Coach begins in-person and / or virtual meetings with young adult & assesses the wellness needs

Coach designs wellness plan. Coach and client identify SMART Goals (daily, weekly, monthly)

Wellness plan is implemented

Weekly call with care team and family to discuss & assess progress

Family calls / emails as needed
PHASE 3

Coach continues to meet with young adult and assess ongoing wellness needs

Coach supports the young adult's wellness plan and SMART Goals

Coach and family review SMART Goals and wellness plan

Adjustments are made to plans and goals as necessary
