PHASE 1

  • ​ Host call with parents of at-risk young adult

 

  • CWC services are thoroughly explained, and a wellness path is curated

  • Case management initiated and coach engagement is activated

PHASE 2

  • Coach begins in-person and / or virtual meetings with young adult & assesses the wellness needs 

​​

  • Coach designs wellness plan. Coach and client identify SMART Goals (daily, weekly, monthly)

​

  • Wellness plan is implemented

​​

  •  Weekly call with care team and family to discuss & assess progress

​​

  • Family calls / emails as needed

PHASE 3

  •  Coach continues to meet with young adult and assess ongoing wellness needs

​​

  •  Coach supports the young adult's wellness plan and SMART Goals

​​

  • Coach and family review SMART Goals and wellness plan

​​

  •  Adjustments are made to plans and goals as necessary

​​

  •  Weekly call with care team and family to discuss & assess progress

  • We collectively determine if services are to be extended
